Monitoring Lung Texture Variations During Lung Cancer Radiotherapy

Purpose: Texture variations of healthy lung tissue during radiotherapy observed by Cone Beam CT might be an early indicator of patient specific lung toxicity. This study investigates the time course of lung texture variations and a possible correlation with locally delivered dose.

Methods: A total of 77 CBCTs in 11 lung cancer patients treated with IMRT to 60Gy in 2 Gy fractions were analyzed. For each patient, features were calculated in 2 dose regions: 5 Gy (low dose) and 20 Gy (high dose), CBCT lung texture variations during the treatment course were related to the locally delivered dose.

Results: 12 intensity-based and GLCM features changed significantly with increasing radiation dose. Individual texture features could vary among patients. Changes at 10th and subsequent fractions are clearly correlated to those observed at the end of the treatment course.

Conclusion: A set of CBCT texture features changes in healthy lung tissue during radiotherapy correlate with the locally delivered dose and can be detected relatively early during the treatment. CBCT could be used to monitor the persistent changes in lung texture, provide a individualized measurement of patient lung tissue reaction to irradiation.
