Purpose: To evaluate the quality management program for proton Stereotactic Radiosurgery (SRS) based on TG 100.
Methods: The studies focus on (1) generating the SRS process map for proton SRS, (2) performing the failure modes and effects analysis (FMEA) and defining the risk probability numbers (RPN) for each sub-process, (3) creating the fault tree analysis (FTA) for the sub-processes with the high RPN scores to identify the possible failures with the high risks, (4) evaluating the quality management program (QM) to check if any possible failures and errors can be propelled into the treatment process.
Results: The general work flow of SRS can be divided into the following steps, (1) initial consultation, (2) simulation, (3) target/structures outlines, (4) plan optimization, (5) plan quality assurance, (6) pretreatment verification, (7) patient setup, (8) treatment delivery. The target/structures outlines, plan optimization and treatment delivery were identified to have the high RPN scores in FMEA. The FTA of the sub-processes listed the possible failures in image fusions, plan images, proton monitor units calibration and dose/monitor units of proton machine. The QM program for proton SRS was evaluated by following the FMEA and FTA. It eliminates possible failures and errors from entering the treatment process.
Conclusion: The quality management program for proton SRS was evaluated from the process map, FMEA and FTA based on TG 100. It guarantees the successful proton SRS treatment.
