Standardization of Nomenclature for Radiotherapy of Head and Neck Tumor in Two Institutions

Purpose: Standardization of structure nomenclature could improve the consistency and the clarity in the radiotherapy (RT) implementation in the national wide. To standardize the structure nomenclatures in head and neck (HN) tumor of RT, we proposed a self-developed software package and validated it in two individual institutions, SYSUCC and HBCH. We aims at the Nasopharyngeal Carcinoma (NPC), not only because it’s the most common HN tumor in the southern China, but also NPC has a complex anatomical structure, an irregular tumor surrounding with vital organs in large quantities.

Methods: 139 RT NPC plans in SYSUCC and 132 ones in HBCH were randomly selected. RT structure files were parsed and analyzed through MATLAB via self-developed software firstly. Secondly, all the structures were classified by the keywords automatically. And then the classified structures nomenclatures were analyzed statistically. Thirdly, the appropriate protocol of different clinical' nomenclatures was introduced based on AAPM TG263. Finally, we programmed the transformation of the nomenclatures and export the structure as a new DICOM RT file, which could be imported back into Treatment Planning System.

Results: The software could automatically standardize the structure names into consistency ones in different institutions. This method was tested and validated effectively in two individual centers. The number of the targets between the two institutions is similar. However, the amount of OARs in SYSUCC is 19.2% more than HBCH, and the derivative and planning structures amount of SYSUCC is 55.6% less than HBCH. There are obviously different nomenclatures preferences, but they could be unified as one after the standardization.

Conclusion: In this paper, the self-developed software was used for difference institutions, providing a consistent nomenclature tool in NPC. Nomenclatures standardization can help to avoid confusion from inconsistency, errors; enable the pooling of data between multiple institutions and improve the quality and safety in treatment.
