Purpose: Development of the first radiobiological simulator calculating mean biological radiation effects (BREs) in one tumor and normal tissue (NT), as well as the tumor control/normal tissue complication probability (TCP/NTCP) for a simulated radiotherapy (RT) treatment using Monte Carlo simulation. This simulator represents a novel methodology, since the TCP and NTCP are calculated without formulas of these indices, but their own probabilistic definitions based on BRE criteria
Methods: The simulations are done using as uncertain variables: the minimum dose per fractions for tumor, and an estimated equivalent uniform dose per fractions for NT, so the other input values used by simulator are considered as well-known.

Conclusion: This application opens new avenues for theoretical and experimental investigation concerning the simulation of treatments in radiotherapy, brachytherapy and interrupted treatment. The acceptable methodologies of RT evaluations will allow establishing methodologies for RT optimizations, which use iterative evaluations aimed to find the best treatments.
