• Fees for registration will be refunded in full if written notice of cancellation is received by Wednesday, March 14, 2018.
• Please send your cancellation in writing to Rachel Smiroldo or fax to 571-298-1230.
• No refund will be given for cancellations received after March 14, 2018.

• If you are paying by check, please make the check payable to AAPM.
• Please mail your check and the registrant's name and copy of registration document to: AAPM

• If you have registered at the Early rate and are paying by check, your payment must be received no later than February 28 (11:59 PM EST).
• If we do not receive your payment for the Early registration fees by February 28, your registration type will be changed to reflect the non-Early registration fee.

Invoices: If you need an invoice prepared for your registration, please send your request to Janelle. Please note that if you have registered at the early rate, we must receive payment on or before February 28 otherwise your registration will automatically change to reflect the late registration fee.
