Purpose: To evaluate the efficacy of three-dimensional arterial spin labeling perfusion imaging (3D ASL) in the radiotherapy of brain metastases.
Methods: Sixteen patients with brain metastases who underwent radiotherapy with routine magnetic resonance simulation before and after radiotherapy, including enhanced magnetic resonance examination and 3D-ASL examination.The mean maximum relative cerebral blood flow (CBF) of the solid part of the tumor was obtained by region of interest (ROIs) method, and the brain blood flow of the opposite mirror area (M), the opposite normal gray matter (GM) and the opposite normal white matter (WM) of the lesion was measured to obtain the standardized tumor blood flow (nTBF)ï¼Œand the differences were compared.
Results: The average CBF values of patients with brain metastases before and after radiotherapy were 48.10Â±5.78 (standard deviation) and 38.38Â±4.38,which was reduced by 20% (p<0.05).When the CBF of tumors was standardized with mirror region (M), contralateral normal gray matter (GM) and contralateral normal white matter (WM) respectively,the standardized blood flow (nTBF) of three tumors obtained by ASL method had statistical significance only in the contralateral gray matter before and after radiotherapy(p<0.05).
Conclusion: As a new non-invasive imaging method, 3D-ASL can be used to evaluate the treatment response of radiotherapy for patients with brain metastases by quantitative detection of neovascularization, which could be applied in adaptive radiotherapy of brain metastases.
