Trans-perineal ultrasound image guidance for prostate radiotherapy: technology, performance, promise and challenges:
Trans-perineal ultrasound imaging is conceptually attractive option for image-guided prostate radiation therapy as there is no need to implant fiducials, no extra imaging dose, and real time continuous imaging and tracking is possible during treatment. We describe the technology behind a trans-perineal commercial ultrasound image guidance system and summarize the findings of phantom studies (including ours) that have examined system performance. We further present experimental designs for evaluating the system intra-fractional imaging and tracking capabilities in patients and summarize the results from corresponding clinical studies at our and other institutions. Finally we discuss the main sources of localization/tracking uncertainties with the use of trans-perineal ultrasound image guidance and summarize research directions towards reducing these uncertainties.
