Purpose: The present study aimed to investigate the efficacy of cancer treatment in IORT technique combined with nanoparticles (NPs), using both analytical calculations and Monte Carlo (MC) simulation.

Methods: The ZEISS INTRABEAM 50 keV IORT machine (ZEISS Medical Technology, Germany) and two of its applicators (1.5 cm and 5 cm diameters) were modelled in the Geant4 MC toolkit. An appropriate physics list was applied to simulate the dose enhancement effects (DEFs) of Silver (Ag), Gold (Au), Bismuth (Bi), Copper (Cu) and Iron (Fe) spherical NPs in different concentrations of 5, 10, 15 and 20 mg/g, at the surfaces of the applicators. The accuracy of simulation was validated by comparing the DEF obtained to that of analytical calculations from tumor vascular endothelial cells (EC) in default dimensions of 20×10×10 µm as a slab of tumor cell.

Results: Simulation results showed that the utilization of NPs led to an increase in DEF up to 20% for IORT combined with NPs. There was a good agreement (±2%) between the results of simulation and analytical calculations.

Conclusion: In this study, the feasibility of using NPs in combination with IORT was demonstrated. Results showed that NPs can enhance treatment outcomes of IORT. We recommend future in vitro and in vivo studies for further insights into this technique.
