Our knowledge and utilization of proton therapy have been rapidly advancing. One area moving particularly fast is artificial intelligence (AI), which is used to facilitate registration, localization, treatment planning, and range verification for proton therapy. Parallel to technological advancement, a better understanding of proton biology is critical. To date, most proton radiobiology studies have focused on in-vitro investigations. In-vivo studies of proton therapy relative biological effect have been limited with a paucity of data on fractionated treatments. The second talk will address the challenges of precision small animal irradiation, focusing on the ongoing development of a novel proton small animal irradiator featuring dedicated solutions of dose delivery and image guidance. The project SIRMIO (Small Animal Proton Irradiator for Research in Molecular Image-guided Radiation-Oncology) aims to provide a portable system for precision image-guided small animal proton irradiation. The system is compatible with existing clinical systems. Its applications in in situ multi-modal anatomical image guidance and in vivo verification will be demonstrated. RBE has been conventionally set at 1.1 for the protons, but the constant RBE may be over-simplification, particularly when it comes to normal tissue sparing. With emerging clinical data, improved estimation of the RBE in the normal tissue and corresponding reduction of the biological hotspots become an important topic of research and clinical practice.
