Altered Volume of Hippocampus Subfield in Pediatric Bipolar Disorder Patients Revealed by Structural MRI

Purpose: plays a key role in affection and cognition of bipolar disorder. The objective of the present study was to investigated changes of hippocampal subfields volume in patients with manic pediatric bipolar disorder(PBD) and euthymic PBD and to provide biomarker for diagnose of PBD.

Methods: Structural T1-weighted neuroimaging scans were obtained from 20 manic PBD patients, 21euthymic PBD patients and 19 age- and gender-matched health controls(HC). FreeSurfer 6.0 software was used for segmentation to obtain the volumes of hippocampal subfields, and 21subfields in each hemisphere were obtained. One-way ANOVA and Kruskal-Wallis test was performed to analyze alterations of subfield volumes among the three groups, and differences between subgroups were examined further by post-hoc analysis with total intracranial volume, age and gender as covariates. Correlation analysis was conducted between the volume values of the subfields showing significant difference between subgroups and clinical indices.

Results: subregions of hippocampus showed significant different volume between subgroups. Compared with HC group, manic group presented reduced volume in left presubiculum head, left parasubiculum and left hippocampal amygdala transition area, and euthymic group showed increased volume in bilateral hippocampal fissure. Compared with euthymic group, manic group showed reduced volume in left presubiculum head and left parasubiculum. No correlation was found between volume values and clinical indices.

Conclusion: exhibiting altered volume were believed to subserve movement, memory, emotion regulation and so on, which may contribute to the abnormal emotional state of PBD. The findings of the present study provide new detail clues about structural damage of hippocampus in PBD patients, which may be helpful for understanding different psychological state of PBD and developing objective biomarker for diagnose of PBD.
