In April 2019, the American Association of Physicists in Medicine published a Position Statement recommending discontinuation of the use of gonadal shielding. Since then, the statement has been endorsed by a variety of national professional medical imaging organizations including the American College of Radiology (ACR), Australasian College of Physical Scientists & Engineers in Medicine (ACPSEM), Canadian Organization of Medical Physicists (COMP), and Health Physics Society (HPS).
• This recommendation has several technical motivations based on advances both in our understanding of radiation biology and in diagnostic medical imaging. The science motivating the change in clinical practice will be discussed.
• Several hospitals have successfully implemented this practice and a report of the transitional experience will be given. Specifically, the challenges for technologists, the staff most impacted by this change in practice, as well as resources to equip a technologist for success will be reviewed.
• State and federal regulations address the use of gonadal shielding in a variety of ways.
Strategies to discontinue gonadal shielding and special regulatory considerations will be examined.
