Enhanced 4DCT Combined with Image Histology Screening and Quantitative Analysis of Left Ventricular Myocardial Function Changes and Stability Characteristics

Purpose: quantitatively analyze the changes of CT radiomics of left ventricular myocardium (LVM) in cardiac cycle by combining radiomics and electrocardiogram-gated 4DCT (ECG 4DCT) enhanced scan.


Methods: reconstruct the images of 20 phases in 14 patients with who underwent enhanced 4DCT scan at intervals of 5% cardiac cycle. The LVM and the well-filled area of left ventricular contrast medium were delineated one by one, and the sphere cardiac cavity ROI with a diameter of 13 mm was delineated one by one. The 3D Slicer software was used to extract 92 features of all the sketches and analyze the distribution of CT values on the cardiac cavity ROI and LVM. The stable features were obtained by preliminary screening of the cardiac cavity ROI. Then the stable features were used to further screen the LVM to obtain the different features, and then the statistical analysis was used to quantify the changes of all the features.


Results: mean CT value of cardiac cavity ROI in cardiac cavity changed less than that in LVM, with the change rates of 9.23% and 17.88%, respectively. There were 36 stable features with no significant difference in cardiac cavity ROI (P > 0.05), and for these 36 stable features, 20 different features were screened on LVM (P < 0.05), and the average change rate was 98.63%, such as Mdian (-103.96%) and Mean (-123.67%) of Firstorder matrix, Gray Level Non-Uniformity (99.81%) of GLDM matrix.


Conclusion: combination of radiomaics and enhanced ECG 4DCT provides a new basis for judging LVM function.The characteristics of Mdian, Mean and Gray Level Non-Uniformity are helpful for monitoring early myocardial damage.
