Purpose: To visualize the transverse plane of meniscus of the knee joint in order to accurately determine the location of the pathology. Also, to reveal injuries that cannot be identified using the conventional techniques.
Methods: 200 subjects, who underwent a special 3-Dimensional Proton Density Fat Saturation (3D-PD FS) MR sequence on of their meniscus in order to reconstruct the transverse images, were examined. 100 of those subjects were healthy and their images were used as part of a qualitative evaluation to verify that all the part of meniscus can be properly visualized. The evaluation was based on a four level scale regarding the visualization of 1: one of the meniscus horns; 2: body of meniscus; 3: body and one of meniscus horns; 4: body and both meniscus horns. On the 100 subjects with pathologies, a qualitative evaluation was also performed based on how well the injury and its location can be visualized. This evaluation was based on a five level scale.
Results: In all the subjects, who had a healthy or pathological meniscus, all the parts of their meniscus could be clearly visualized. The homogeneity, the contrast between meniscus and its contour and the accuracy of tear topography absolutely improved in the reformatted images by the 3D-PD FS sequences. The produced images were closer to real arthroscopic images and they presented much better the root and the radial meniscal tears. The contrast of meniscus with contour structures was very satisfying in both planes of reformats, compared to the conventional sequences.
Conclusion: We propose the use of the 3D-PD FS sequence because it provides the possibility of transverse reconstruction in knee MR examination for the visualization of meniscus. It was shown that those sequences can provide high definition quality images, which can accurately illustrate the injuries of meniscus and their location.
