The registration fee is $375 and includes lunch and coffee breaks on both days. Registration is limited, so do not delay.
Once we have reached maximum capacity, you may register for the waitlist. You will be asked for credit card information, but your card will not be charged unless a registered participant cancels, allowing you to move up to the available slot.

Cancellation Policy
Fees for registration will be refunded in full if written notice of cancellation is received by October 1, 2019. Please send your cancellation in writing to Jaime Hoza. No refund will be given for cancellations received after October 1. Registrations received after October 1 are non-refundable.

If you are paying by check, please make the check payable to AAPM. Please mail your check and the registrant's name and copy of registration document to arrive no later than October 1 to:
