Diffusion Parameters of Fornix in Multiple Sclerosis Patients with Memory Disturbance

Purpose: sclerosis (MS) is one of the most common debilitating disorders. MS presented by distinctive signs and symptoms, including cognitive impairment, which affects about 40-70% of patients. One of the most important types of cognitive impairment is memory complaints. Various studies have shown numerous changes in the brain structure of MS patients. In this study, the relationship between cognitive impairments and structural changes of fornix, as an cognitive pathway was investigated in MS patients.


Methods: present study was performed on 10 normal subjects (NL group), 16 individuals with MS but without cognitive impairment (MS-CI group) and 11 individuals with MS and cognitive impairment (MS+CI group). In order to assess the extent of memory impairment, Wechsler test were used. On the other hand, fornix was evaluated by Diffusion Tensor Imaging (DTI) technique.


Results: data showed that fractional anisotropy (FA) of fornix decreased significantly in MS+CI group compared to NL (p?? < 0.01) and MS-CI (p?? = 0.05) groups. Mean diffusivity (MD) was increased remarkably in MS+CI group in comparison to NL and MS-CI groups (p?? < 0.001). Also results revealed that there was a strong relationship between elevation in MD and reduced FA in the fornix tract and the presence of cognitive impairments.


Conclusion: MD changes in fornix of MS+CI group was higher than FA. Signigficant correlations between disability in perform Wechsler test and structural changes in fornix may represent demyelination severity and disturbance in axonal alinement of fornix in MS patients with memory loss.
