Purpose: high quality radiation treatment plans for head and neck(HN) cancer requires extensive training and experience due to the elaborate anatomic features in HN area. One particular challenge is knowing what constitutes ‘good’ contours or dose distribution. The goal of this project is to develop an external beam radiation oncology treatment plan library(ROTPL) that can provide clinical teams easy access to pre-selected, high-quality plans from matched patients that were previously treated at our institution.

Methods: relational database was designed using Microsoft Access to connect patient information, diagnoses and radiation treatment plan details. A Matlab interface was developed to search/retrieve HN plans based on treatment sites, patient information, radiation technique, dose prescription, and planning parameters. The interface integrates 3D Slicer, an open source program, for contour and dosimetry review. Statistics of target and organs-at-risk(OARs) can be calculated for any retrieved group/category to identify standardized clinically achievable goals of high quality plans based on optimized target coverage and normal tissue sparing. The current iteration includes a total of 115 select high-quality IMRT/VMAT plans (40 routine HN plans and 75 HN stereotatic treatment plans) previously optimized by experienced dosimetrists and physicists in Pinnacle or RayStation. These plans were classified into 9 categories based on treatment sites.

Results: ROTPL was evaluated and tested by the HN physicians and physicists to search and review high quality treatment plans. The statistical information from these high quality plans provided standardized dosimetry results for physicians when considering OAR dose constraints, and provided planners a benchmark of achievable clinical goals for best practice.

Conclusion: ROTPL is a great tool for education and training, and can serve as a reference in HN treatment planning process. This tool can doubly serve as an electronic handbook of high quality plan generation and evaluation for radiation therapy professionals.
