Sir-Sphere Dosimetry Calculation Based On Tumor Volume

Purpose: The development and application of Yttrium-90 radioembolization represents a significant advancement in the treatment of hepatocellular carcinoma and liver metastasis. Currently, the Sir-sphere activity for the management of liver metastasis is calculated with a formula based on the body surface area and tumor involvement. The tumor involvement is defined as the ratio of tumor volume to liver volume. The purpose of this study was to review and analyze data, including tumor volumes, liver volumes, height, weight, and body surface area, and aims to find a potential correlation between tumor involvement and other variables retrospectively. The study also tries to find an alternate way of calculating tumor involvement by correlating it to other variables to reduce activity calculation time.

Methods: 200 patients with liver metastasis treated with Sir-sphere from 2013 to 2019 were included in this study. The tumor involvement, height, weight, liver volumes, tumor volumes of each patient were analyzed to determine if a correlation relationship exists between tumor involvement and these variables using Pearson’s correlation.

Results: A Pearson’s correlation analysis yielded a correlation coefficient of 0.921 between tumor involvement and tumor volumes, and indicated a strong correlation between tumor size and tumor involvement. A linear regression analysis between tumor involvement and tumor size showed an R-squared value of 0.847 and a p-value < 0.05. There is no correlation between tumor involvement and height, weight, or body surface area.

Conclusion: This study shows a strong correlation relationship exists between tumor involvement and tumor size. Tumor involvement can be estimated based on the tumor volumes. The liver volume could be excluded from the Sir-sphere dosimetry calculation. This would dramatically reduce the time spent on segmentation, thus increase the efficiency in the activity calculation using Sir-sphere.
