Purpose: 3D printing technology has been adapted into our clinic for machine quality assurance and also patient care assistant for many years. BolusVision is one of the major applications. It creates 3D photon/electron bolus by printing rigid molds for soft bolus casting. BolusVisionOnline is further integrated with Eclipse Scripting API (ESAPI, Varian Medical Systems) to highly improve 3D soft bolus fabrication clinical workflow.

Methods: The proposed BolusVisionOnline is a tool to automate the mold generating and printing in ESAPI. It took two steps to achieve the goal. (a) Creating enclose negative mold: expanding the bolus structure by 5 mm then subtracting the original 3D bolus structure. This operation creates a 5 mm thickness shell from the desire bolus structure. (b) Parting the negative mold into inward and outward pieces: By performing an AND Boolean operation between the negative mold and BODY structure to acquire the inward piece mold. After subtracting the inward piece mold structure from the original negative mold to acquire the outward piece mold structure. Lastly, two STL files are generated for these two piece mold structures for 3D printing.

Results: The advantages of using ESAPI script to create STL files for mold structures include (1) built-in 3D mesh in Eclipse for structures, (2) creating a parting line of the mold is done by using Boolean operations between structures, (3) no extra DICOM files transferring to external standalone software, and (4) workflow efficiency improvement. The processing time is approximately within two seconds.

Conclusion: An ESAPI script has been developed for 3D bolus mold printing. It has improved the efficiency significantly and sped up the 3D soft bolus fabrication process.
