Analysis of Patient Treatment Time for Adaptive Work Flows On the Elekta Unity

Purpose: and analyze the time required for adapt-to-position (ATP) and adapt-to-shape (ATS) workflows using the Elekta Unity and identify the driving causes for variation in total treatment time.


Methods: a single month of treatments, the total treatment time was manually recorded as well as the start/finish times for patient setup, imaging, planning and review, and treatment delivery. Any events not related to the standard workflow which impacted treatment times were also recorded including machine issues, patient related issues, and physician unavailability. The effect of these recorded events on the average treatment time was investigated by comparing the mean treatment times between cases with and without events.


Results: results were acquired for 111 ATP fractions and 35 ATS fractions. The mean and standard deviation of the total treatment time was 38±10 minutes for ATP workflows and 55±16 minutes for ATS workflows. All timed components between ATP and ATS where within 1.1 minutes except for the planning and plan review time where ATS took 17 minutes longer on average. When excluding events outside the standard workflow, the total treatment time for ATP and ATS were 34±4 minutes and 53±12 minutes respectively.


Conclusion: on the one-month sample of treatment delivery times, between 12 and 13 patients can be treated in a 9-hour workday with an average treatment time slot of 42 minutes per patient. Efforts to decrease the total treatment time to improve patient throughput should be focused on the planning and plan review time as there is minimal room for improvement elsewhere in the workflow. Future timing work will be aimed at further investigating the planning and review time to determine the additional workflows steps to optimize.
