Gout Phantom for Dual Energy CT

Purpose: purpose of this study is to construct a gout phantom comprised of relevant materials that can be used for algorithm development for dual energy CT (DECT).

Methods: gelatin-based, gout phantom was created with uric acid (UA) gout inserts of 3 sizes (10,7,2 mm³) and 5 concentrations (10-50% UA by mass). Sizes were chosen because these represent the mean crystal load of gout patients in the feet (10 mm³), hands, knees, and elbows (7 mm³), and the minimum limit of detectability for gout by DECT (2 mm³).
Pseudogout, bone, and tendon were also included as these materials are sources of false positives (tendon and pseudogout) and false negatives (bone) in DECT gout assessment. Pseudogout can be confused with gout when only CT # is used but can be separated by material on DECT. Pseudogout inserts of the same size and concentrations as gout inserts were included. Chicken bone and cow tendon were used. All materials were arranged in tissue-like gelatin.

Results: inserts in the phantom fell within clinically relevant ranges on dual energy CT material images whereas the pseudogout inserts were not within the thresholds.

Conclusion: work shows the potential for using a gout specific phantom as ground truth for DECT gout algorithm development, specifically with regards to distinguishing gout from pseudogout.
