Stereotactic Ablative Radiotherapy (SABR) has been recently investigated as a potential non-invasive treatment technique for cardiac arrhythmias such as ventricular tachycardia (VT). Despite promising initial clinical results, target location and motion management remain major challenges for widespread clinical implementation of this emerging treatment technique. Conversely, radiation-induced heart disease has been linked with increased morbidity in patients with breast and lung cancer that received radiation treatment, prompting special consideration of heart tissue sparing for radiation treatment planning. The goal of this educational session is to describe the current state of the art of SABR for cardiac VT and discuss the strategies for heart dose sparing in radiation treatment planning. An overview of current clinical studies and outcomes will be provided, followed by presentation and discussion of strengths and limitations of various imaging techniques for target localization and motion management. Lastly, strategies for dose estimation and cardiac dose sparing for radiation treatment planning will be reviewed and discussed.
