Developments and implementations of image-guidance technologies leads to image guided radiation therapy, delivering radiation dose more precisely to tumor while sparing adjacent healthy tissue. Latest progress to integrate quantitative imaging into radiation therapy (RT) enables quantitative treatment assessment in RT.

In the new era, numerous efforts are gaining momentum by advancing cancer care through individualized or precision medicine, which applies quantitative imaging techniques to determine RT treatment response. These metrics can be used to predict a patient’s outcome and optimize treatment strategy. Research efforts have demonstrated that quantitative imaging is a valuable tool to assess treatment response of RT. These tools, however, present with challenges and pitfalls including image artifacts, lack of reproducibility, registration uncertainty, protocol standardization and intensive computational analysis, which impose limitations and cautionary use when branching out from the research domain into standard clinical practice.

This lecture will provide an overview of quantitative imaging techniques including MRI, PET and hybrid PET-MRI, methodologies and applications of quantitative imaging, as treatment feedback, to assess treatment response in RT, and challenges and pitfalls using quantitative imaging for RT response assessment.
