Performance Evaluation of a Digital PET/CT Based On NEMA NU2-2018 Protocol and Jaszczak Phantom Analysis: Comparison Between 68Ga and 18F

Purpose: recent digital PET detectors technology may improve diagnostic capabilities and patient care by providing higher image quality. Our objective is to evaluate performances of Philips VEREOS digital PET/CT, using an in-house software, and provide a comparison between fluorine-18 (18F) and gallium-68 (68Ga) acquisitions.

Methods: 18F and 68Ga sources, we evaluated spatial resolution, sensitivity, image quality, scatter fraction and noise equivalent count rate (NECR) according to NEMA-NU2-2018 procedures. We also evaluated image quality for the two isotopes using a Jaszczak phantom according to the methodology described in the work of Hirtl et al (2017) and AAPM TG126, providing visibility index (VI), standard deviation to mean ratio (SMR) and contrast to noise ratio (CNR). To validate our software, the results were compared to those obtained from VEREOS software when available.


Results: analysis shows very close results between 18F and 68Ga acquisitions. Main results are the spatial resolution values ranging from 4.03mm to 5.79mm (18F) and from 3.97mm to 6.11mm (68Ga) and image quality indices measured with the Jaszczak phantom. Especially, mean VI, calculated from the spatial resolution sector for six groups, were ranging from 1.07 to 0.19 for 18F and from 1.04 to 0.18 for 68Ga and the CNR values of the 6 spheres were measured from 12.1 to 7.4 for 18F and from 13.5 to 7.8 for 68Ga. The SMR values were 7.4% for 18F and 6.7% for 68Ga.


Conclusion: software successfully firstly assessed the performances from phantom images and was validated against the vendor software results and the reference NEMA-NU2 performances of the PET system. Then, 68Ga and 18F sources images analysis showed very similar performances. The spatial resolution and the VI were slightly improved for 18F compared to 68Ga.
