Dosimetric Impact of Leaf Interdigitation in Whole-Brain Irradiation with Simultaneous Integrated Boost VMAT Treatment Planning

Purpose: To retrospectively analyze the dosimetric impact of whole-brain irradiation (WHRT) with simultaneous integrated boost (SIB) by leaf interdigitation VMAT treatment planning in the treatment of multiple brain metastases (BMs).

Methods: 25 previously treated BMs (no less than 3 lesions) patients were re-planned for VMAT with and without leaf interdigitation using Monaco 5.1 TPS on the Elekta Synergy linear accelerator. A dose of 40 Gy was delivered to the whole brain in 20 fractions, while a dose of 60 Gy was delivered to the gross target volume (GTV) in 20 fractions. Several dosimetric variables, such as target coverage, OAR sparing, delivery efficiency and optimization time, were evaluated for each plan.

Results: Interdigitation did not improve the coverage, HI and CI for target. Regarding OARs, sparing was equivalent with and without interdigitation. However, interdigitation shown an increase in MUs. Meanwhile, leaf interdigitation saved the optimization time.

Conclusion: This study shows that leaf interdigitation does not improve plan quality when performing VMAT treatment plan for BMs patients. However, it influences delivery efficiency and optimization time. Interdigitation may gain efficiency for dosimetrist when designing the BMs VMAT plans.
