TG119 Validation of a Novel Treatment Planning System with Auto-Planning Workflow Utilizing a Flattening-Filter-Free, Ring Gantry Linear Accelerator

Purpose: Validation of a novel treatment planning system (TPS) using the AAPM TG119 intensity modulated radiation therapy (IMRT) commissioning tests. The prototype TPS incorporates a unique auto-planning workflow for IMRT with either static gantry or arc delivery.

Methods: Five anthropomorphic treatment plans (multi-target, mock prostate, mock head and neck, C-shape easy, and C-shape hard) were generated using the provided datasets using the auto-planning workflow and prototype optimization platform. Plans were validated on the ring-gantry system using an ionization chamber (0.125cc active volume) placed isocentrically within a solid water phantom (15cm x 15cm x 15cm) to confirm absolute dosimetry. Portal dosimetry was used as a 2D verification of plan deliverability (150cm SID). Gamma analysis was used with 3% 2mm and 2% 1mm thresholds and results were tabulated in the TG-119 data form in order to yield a quantitative confidence limit.

Results: Portal dosimetry gamma criteria passing rates were well within clinically acceptable tolerances. Absolute dosimetry was confirmed to within 2% for point dose measurements inside the PTV. Confidence limits established via TG119 criteria were within expected clinical values when compared with conventional C-arm linacs.

Conclusion: Pre-clinical testing of the prototype TPS was validated using TG119 and found to be clinically acceptable for all test plans. This new TPS, in conjunction with the ring-gantry treatment delivery system, is a viable radiotherapy platform and permits fast, highly automated IMRT planning and treatment delivery.
