Pencil Beam scanning is now widely used at many proton therapy centers. This development is accompanied by advances in delivery and treatment planning. One of two recent advances on the treatment delivery side is the introduction of automated gating in combination with voluntary breath hold. The technical implementation and clinical benefits of this technology will be discussed. The second advancement on the delivery side is the use of proton technology for grid treatments. This presents the marriage of an older treatment concept with spot scanned proton beams. The dosimetric advantages of this technique will be discussed.
Two advances on the treatment planning side will be presented next. Firstly, the use of the proton linear energy transfer (LET) in treatment planning and optimization will be discussed. The implementation of the data aims to minimize the high LET in nearby critical organs to avoid
possible high-LET-induced toxicities and reduces the biological uncertainty that is inherent to a proton therapy delivery at the beam’s track end. The last speaker will introduce model-based treatment planning for proton therapy. While it is frequently used in photon planning it is a new deployment in proton therapy.
