Initial Experience of Per Fraction Software in a Community Setting

Purpose: To implement an additional method of performing patient specific QA for IMRT patients as well as a monitoring system using the log files to asses continuous Linac performance.

Methods: The Per Fraction software from Sun Nuclear was installed to work in conjunction with an ELEKTA VERSA HD (2016) Linac. Step by step procedures will be presented to show how the software was installed to work with the Pinnacle treatment planning and MOSAIQ software. Also, the specific calibration procedure for the EPID with the Per Fraction and MOSAIQ software will be explained.Two VMAT patient plans (pre treatment QA) were delivered to the EPID with the PerFraction software activated. This delivery in air (called fraction zero F0) was compared with the results of similar measurements done with the ArcCheck. For the same two patients the plans were checked with the Radcalc and Sun Nuclear Dosecheck software. Finally, the same two patients complete treatment courses (fraction N FN)were followed to assess the log files from the Linac.

Results: Per Fraction point dose, 2D analysis (gamma) and 3D analysis (DVH) results for fraction zero (F0) were all within the typical 3% 3mm criteria. The log files percentage agreement for all fractions (FN)were within 1% of each other

Conclusion: The Per Fraction software present clear advantages when compared to typical devices used for patient specific QA such as the Matrixx or the ArcCheck. There is no QA plan or need to use of any other device that is placed on the treatment room besides the EPID. The software run in web base environment, meaning it could be assessed from any computer. Per Fraction also has the potential to be used during the actual treatment to account for patient anatomy changes or Linac malfunction.
