Development of Novel Individual Applicator for High Dose Rate Brachytherapy for Superficial Malignant Lesions

Purpose: The aim of this study is to develop novel individual applicators which can be soft and flexible to enhance the adhesion with the superficial lesion to improve the treatment accuracy of HDR brachytherapy.

Methods: The CT image set for head phantom was acquired on a Brilliance CT Big Boreâ„¢. The virtual applicator was contoured for irregular surface such as nose using CT data set. The applicator had 1 cm thickness and 7 source paths in applicator were positioned at 1 cm interval and 0.5 cm away from surface. Two individual applicators were made using 3D printing technique and compared with commercially available Freiburg flap (FF_App). One was printed using UV resin which has physical density of 1.02 g/cc (Hard_App) and the other was to make applicator mold. The dragon skin (silicone, physical density: 1.07 g/cc) was poured into the mold to make the soft and flexible applicator (Soft_App). Treatment planning for FF_App, Hard_App and Soft_App was performed using the Oncentra Brachytherapy treatment planning system with 192 Ir source. Three measurements at each applicator were performed with EBT3 films to evaluate reproducibility of measurement.

Results: We fabricated novel individual applicators by using 3D printing and by pouring the dragon skin into the 3D printed mold compared with commercialized applicator. The Soft_App was more adhered to the curved skin due to soft and flexible properties as compared to the FF_App and Hard_App. The two individual applicators showed better reproducibility of the measurement. In particular, Soft_App improved 18.8% and 5.9% in the 2% / 2 mm criteria compared to FF_App and Hard_App.

Conclusion: Novel individual applicator for HDR brachytherapy was successfully fabricated and the reproducibility of the measurement was improved compared to commercial applicator. Especially, the Soft_App which is more flexible and well-fitting on the surface showed better reproducibility than the Hard_App.
