Pediatric nuclear medicine imaging and therapy present a unique set of challenges that arise from the diverse and often non-complacent patient population. The therapy component is further complicated by the scarcity of published standards and information on activities needed to treat pediatric patients of various ages and weights. This symposium presents several aspects of pediatric nuclear medicine therapies: I-131 thyroid ablation, I-131 MIBG, Sm-153 EDTMP, Y-90 microsphere liver embolization and Lu-177 DOTATATE, at dedicated pediatric and adult hospitals. It will include guidance on how to start a new nuclear medicine therapy program for pediatrics and the pitfalls encountered with the first few patients. Lessons and musings from a mature program will provide information on how to improve the safety and efficacy of pediatric therapies through patient-specific, quantification-based activity prescription. The session will conclude with the perspective of an adult hospital that occasionally treats pediatric patients.

Jay Poston - Lessons Learned from Pediatric I-131 Therapy
This lecture will describe the preparation needed to start up pediatric nuclear medicine therapy programs, with emphasis on I-131 MIBG pre-planning and lessons learned from the first few patients. It will also include interesting thyroid ablation cases and a notes on how to start a pediatric Y90 program.

Sam Brady - Quantification-based Pediatric Theranostics
This lecture will outline the next step in the evolution of a pediatric theranostics program through quantification-based pediatric therapy. It will present similarities and contrasts to start up programs through lessons learned after decades of implementation.

Bill Erwin - Pediatric NM Therapy in an Adult Hospital Setting
This lecture will present the experiences of an adult hospital that occasionally treats pediatric patients with I-131 NaI, Y-90 microsphere and I-131 MIBG . Application of both established and upcoming therapeutic agents such as dosimetry-based high-activity I-131 NaI and Sm-153 EDTMP, Azedra I-131 MIBG, Xofigo Ra-223 chloride, and Lu-177 DOTATATE in a pediatric population will be described.

Learning Objectives:
1. To understand the process of starting a new pediatric theranostics program, from site visits and shielding design to lessons learned from the first few patients.
2. To learn how quantification-based nuclear medicine therapies can be customized to the pediatric patient and improve therapeutic outcome.
3. To describe the challenges encountered by adult-only hospitals when occasionally treating pediatric patients.
