Evaluation of the Effect of the Target and the Overlap Volume of the Organ On the Automatic Optimization of the Knowledge-Based IMRT Plan for Cervical Cancer

Purpose: Through the comparison of 20 cases of patients with cervical cancer after surgery (part of the bladder and rectum PTV target overlap) K-IMRT dosimetric differences between Knowledge-based IMRT(K-IMRT) and Conventional IMRT(C-IMRT) plan to investigate the feasibility of cervical cancer K-IMRT plan automatic optimization.

Methods: 110 patients with cervical cancer were treated with intensity modulated radiotherapy. 100 cases of patients as the model for the establishment of automatic optimization model. Out of model group, 10 cases of the target area and the organ with overlap volume as the experimental group E1 (open-loop verification).In the model group, the target area of the 10 patients with the volume as the experimental group E2(closed-loop verification). E1 and E2 groups were used automatic optimization model to generate K-IMRT plan respectively and compared to the C-IMRT groups of dosimetric differences.

Results: Compared with the target dose parameters of the two groups, there was no significant difference between the target dose parameters of the E1 group and the minimum dose and HI of the E2 group (P > 0.05).The maximum dose and CI of E2 group in K-IMRT group were significantly better than those of C-IMRT intensity modulated plan, the difference was statistically significant (P < 0.05).Compared to the parameters of organs at risk in C-IMRT plans, for E1 group of rectal V20, left and right femoral head V15, V20, V25; V25, V30, E2 group of bladder V35, D50, V25, V30, rectum, left and right femoral head V15 V20, V25 , K-IMRT was significantly reduced. The difference was statistically significant (P < 0.05).

Conclusion: It is feasible to optimize the intensity modulated radiotherapy using knowledge-based plan model for patients of cervical cancer with the overlap volume of target and organs.
