Dosimetric Comparison of RapidPlan and Manually Optimized Plans in IMRT for Postoperative Left Breast Cancer

Purpose: This study evaluated whether RapidPlan based plans (RP plans) created by a single optimization, are usable in intensity modulated radiation therapy (IMRT) for patients with left breast cancer.

Methods: We used 56 previously administered IMRT plans to train a RP model. And then, another 20 cases of postoperative breast cancer patients were selected, using the DVH estimation model to design the plan (RP_IMRT), which required consistent with the model plans. Differences between RP plans and clinical manual optimization plans(MP_IMRT) created by an experienced planner for the same patients were analyzed.

Results: No statistically significant differences were observed in target homogeneity index (HI) and conformity index (CI) between RP_IMRT and MP_IMRT plans. RP_IMRT plans showed the lower values in V20 and the mean dose of left lung. For the mean dose in heart and V30, no statistically significant differences were observed. The time spending on RP_IMRT was shorter significantly. The monitor unit (MU) was not statistically significant.

Conclusion: In this study,the RapidPlan was used in the intensity modulated treatment plan of postoperative left breast cancer, and the dose of OAR was significantly reduced. The simple model could reduce optimization time and improved work efficiency., independently of planner's skill and knowledge.
