Purpose: The conventional IMRT planning is tedious and repetitive. Its outcome depends on the planner performing the optimization. The study is to investigate the performance of auto-planning in Pinnacle3 for lung cancer with step and shoot intensity-modulated radiation therapy (ssIMRT).

Methods: 15 previously treated lung cancer patients were re-planned for ssIMRT using auto-planning in Pinnacle3. The PTV of lung cancer contained gross target volume (GTV) and the positive lymph nodes. The volume of PTV ranged from 177 cm3 to 596 cm3. We compared the manual planning and the auto planning using DVH metrics.

Results: Auto-Planning could generate clinically acceptable treatment plans for all the cases. The PTV coverage and the OAR sparing are similar in the two types of plans. The MUs of auto-plans were slightly less than the manual plans. It's worth noting that auto-plans could reduce the optimization time and increased the optimization efficiency.

Conclusion: Our study demonstrates that auto-planning can produce clinically acceptable treatment plans. It makes IMRT process faster and efficient.
